All new clients should download the New Client Packet by clicking below. This includes two documents. One of them is our disclosure that discusses confidentiality and other information about working with LifePaths Counseling. Please print this document, review the content, complete and sign it, and bring it with you to your first session. If you aren’t able to print this, we have copies in the office as well.
In addition to the New Client Packet, please see below for additional forms if you are a couple seeking counseling, or if you are seeking counseling for a minor.

If you are bringing your child to counseling, please complete the form linked below for your child and bring it with you to the first session.
If you are or are bringing your adolescent to counseling, please download the forms linked below. The first is an information sheet. You don’t need to complete this form AND the one included in the New Client Packet, just this one.
If you are a parent bringing your minor (non-emancipated) teen in for counseling, please review this second form linked below. As the parent of a minor, you may be entitled to some information and records about your child’s treatment, depending on their age. However, many times a teen needs to be assured that he or she can speak with a counselor privately in order for treatment to be most beneficial. This form asks for your cooperation in allowing for that level of confidentiality. Please know that we also encourage teens to share as much as possible with you, and that if your counselor feels that your teen is in danger or it is in his or her best interest to discuss concerns with parents, we will not hesitate to do that.

Occasionally during our psychotherapy sessions, your counselor will refer you to this page to download one of the documents linked below. Please see the descriptions with each form.
★ Cognitive Distortions – this document lists several common ways that we can get stuck in non-productive ways of thinking.
★ Change Negative Thinking – this document lists several ways you can challenge negative thinking and bring positive change to your life.
★ Relationship Communication – handout for clients on ideas to improve communication and conflict resolution skills in a relationship.
★ Boundaries – handout for clients on maintaining healthy boundaries with others.
★ Betrayal and Rebuilding Trust – handout for clients on what happens when one partner betrays the other, types of betrayal, and how to rebuild trust afterwards.
★ Healthy Relationships – this handout is a collection of ideas of what a healthy relationship is likely to look like. It can help you identify areas where you can work on your relationship, or serve to validate how well you think the two of you are already doing. There are also resources for additional reading.
★ Grief – these are a few ideas about what a person can and cannot do as a part of the grief process. This is created from material found in Transcending Loss by Ashley Davis Bush, LCSW.
★ Self-Esteem – this handout includes resources and ideas to get started working on self-esteem.
★ Anger Management – This is a client handout for initial work on anger management.
★ Self-Care – This one is a collection of ideas of things you can do to take care of yourself, reduce stress, and find healthy coping skills that work well for you.
